If you are a senior or disabled adult living at
home in Scarborough and you need on-going assistance with activities of
daily living, this program may be for you. Supportive Housing is
designed to provide you with a comprehensive plan of personal support
and homemaking assistance. The program is flexible and is designed to
compliment any assistance you may be already receiving from friends,
family members and peers.
The program provides assistance in the following areas:
First, we visit you along with your family
and/or caregiver to assess your specific needs. Then, together we
develop a personalized Care Plan for you. As part of your Care
Plan we set up a schedule of regular visits by our Community Support
Workers (CSWs), who help look after your needs. You will also receive
regular visits by the Program Manager to ensure your plan is running
smoothly. The CSWs provide regular feedback to the Program Manager to
ensure that changes are made in your Care Plan to meet your changing
needs.
This Supportive Housing Program is provided at a designated location in
Scarborough.
There are no fees for the services offered by
the Supportive Housing Program.
